Conduct outreach in the community to engage churches, organizations, and individuals to promote the services of HealthVisions and develop partnerships in the work and help with volunteer and client recruitment.
Does presentations and assists with being a liaison with the community along with the Director.
Complete screenings and assessments of persons who are at risk or identified as socially isolated and lonely to enroll persons in the Circle of Friends, and Telephone Reassurance programs.
Work with churches to develop Circle of Friends and reassurance calls ministry to reduce social isolation.
Follow up with persons who receive services or have been referred for services to assure that their needs are met in the appropriate websites, databases, and referral logs.
Document according to policies in the appropriate files and databases.
Identify participants to continue meeting with the group and provide support after the scheduled sessions end.
Train and mentor the church facilitators of their Circle of Friends, outreach, and reassurance call ministries.
